A minor, age 16 or older, may petition the court to become emancipated. There is a $150.00 filing fee. The following forms must be submitted at the time of filing of the petition:
- Petition for Emancipation, Affidavit, and Waiver of Notice (pc100.pdf)
- Summons (pc79.pdf)
- Order Following Hearing on Petition for Emancipation (pc101.pdf)
- A certified copy of the minor’s birth certificate.
The affidavit on the back side of the petition must be completed by one of the following individuals:
- Member of the clergy.
- Family therapist.
- Certified social worker.
- Social worker.
- Social work technician.
- School administrator.
- School counselor.
- Law enforcement officer.
- Duly regulated child care provider.
