Changes in Personal Information

ADDRESS CHANGES

Address changes can only be made in writing to the Friend of the Court. Every court order provides that changes in addresses must be made to the Friend of the Court within 21 days. Changing your address at the MiSDU, or US Bank, or the U.S. Post Office does not change your address with the Friend of the Court. To change your address with this office you must send in a written notice dated and signed or complete the change in personal information form paying attention to line 9, dating and signing the form. NAME CHANGES

Names changes can be completed by submitting the change to our office in writing attaching verification (order, marriage license,) to the Friend of the Court or complete the change in personal information form paying attention to line 9, dating and signing the form. (FOC 108)

EMPLOYMENT CHANGES

Employment Changes can be taken over the phone, in writing or by completing the change in personal information form, paying attention to line 9, dating and signing the form (FOC 108).
